What we are looking for?


Council is looking for a Team Leader Access & Social Support within the Family Health Support Services Department who will be responsible for the delivery of Home Maintenance, Community Transport, Planned Activity Group programs and Intake Officers.  Your main responsibilities will include: 


  • This challenging role will ensure day to day responsibility and operational leadership of the Home Maintenance (HM), Community Transport (CT), Planned Activity Group/Social Support Group (PAG) and Intake team members.
  • Undertaking service programming in line with Client goals and feedback, within the requirements of the Home and Community Care Program for Young People (HACC-PYP) guidelines and the Commonwealth Home Support Program (CHSP)
  • Lead, motivate, supervise and assist team members in an open and co-operative team environment demonstrating corporate values


About you


  • You will be a highly engaging leader that can successfully motivate team members across a diverse range of programs
  • You will have a firm understanding of the needs of older residents and experience in planning, developing, implementing and evaluating activities and programs will be required for this role. 
  • Suitably qualified with relevant experience, you are a creative, positive, dynamic and hands on leader, with well-developed communication skills. You have a passion to deliver quality outcomes that will benefit the local community
  • You will thrive in a multi-faceted role with excellent computer literacy including understanding and use of client management systems

Selection Criteria Relevant tertiary qualification with proven experience in Business Administration, Leisure & Health, Community Development or other related field/s
Excellent verbal and written communication skills with the experience in preparing correspondence and promotional material
Experience leading a community focused team to provide an excellent standard of customer service
Excellent computer literacy skills including understanding and experience in client management systems
