What we are looking for?


Council is looking for an Assessment Officer within the Family Health Support Services Department who will be responsible for holistically assessing local residents to identify their needs and provide access to relevant support services and information in assisting them to remain living independently and safely within their homes. Your main responsibilities will include: 


  • Conduct Home Support Assessments using the National Screening Assessment Framework (NSAF) to determine eligibility for the Commonwealth Home Support Programme (CHSP) and establish Client Support Plans
  • Conduct Living at Home Assessments (LAHA) for the Home and Community Care Program for Younger People (HACC PYP) and establish Client Care Plans
  • Undertake Support Plan Reviews and conduct Reassessments of Clients needs within the My Aged Care system in line with priority and My Aged Care Regional Assessment Service Guidelines
  • Provide referrals to internal departments and external agencies and work collaboratively with them to ensure appropriate care for Clients


About you

  • A relevant tertiary qualification in Social Work, Nursing, Occupational Health or other related allied health field.
  • Substantial experience working with complex Clients and undertaking holistic assessments.
  • An understanding of the Home and Community Care Program for Younger People, My Aged Care and Commonwealth Home Support Programme and Wellness & Reablement Approach.
  • A current Victorian Drivers Licence.
  • A current First Aid Certificate
